A medication review involves a consultation with your local Pharmacist to review and discuss the medicines you are taking.
It is particularly beneficial to those recently discharged from hospital, or anyone taking five or more medications regularly.
During the review your pharmacist will provide you with a comprehensive list of your medications including, what they are used for, how they should be taken and stored and how they can affect other medical conditions.
